Iron Blight Sickness Guide: Patch 1.04 Status
Understand official Iron Blight Sickness changes through Patch 1.04, including Hardcore defaults and vodka Quick Use behavior, without unverified meter claims.
On this page+
Sickness is a version-sensitive Iron Blight survival modifier. On the current Patch 1.04 baseline, Steam confirms that vodka is only consumed automatically by Quick Use when Sickness is critically high; it does not publish the exact threshold, meter formula, or every gameplay effect. Plan around the confirmed behavior instead of copying launch-era assumptions.
What is officially confirmed
| Patch | Confirmed change | What to do with it |
|---|---|---|
| 1.01 | Sickness is inactive by default on Hardcore difficulty. | Check your selected modifiers before a challenge run; do not assume every Hardcore run begins with it active. |
| 1.03 | Sickness and low-health effects became more significant; vodka cures more Sickness. | Treat deterioration as a higher priority and keep recovery decisions deliberate. |
| 1.04 | Vodka Quick Use only consumes automatically at critically high Sickness. | Do not expect Quick Use to spend vodka at every lower level. |
Practical recovery plan
- Check your active difficulty and modifiers before leaving the opening area.
- Keep one usable recovery option accessible, but do not turn every vodka pickup into an automatic action.
- Avoid entering a major puzzle, long backtrack, or named encounter when both health and Sickness are already a concern.
- Reassess after an update: Patch 1.03 materially changed both the impact of Sickness and the amount vodka cures.
- Make a separate save before a high-risk route segment, then use Save & Quit guidance at a stable stopping point.
What this page does not claim
The developer's public notes do not give numerical Sickness thresholds, a complete list of symptoms, a fixed vodka quantity, or a verified conversion between Sickness and health. This wiki will not invent a percentage, timer, or “best moment” value from an unlabeled interface observation.
If Quick Use does not consume vodka, first distinguish between a normal state and the critical state explicitly named by Patch 1.04. Then check that your version is current and that the item is actually available to the quick-use system. Record the difficulty, active modifiers, and visible game state before reporting a different result.
Sickness in challenge runs
The difficulty and modes guide explains which challenge requirements Steam publicly confirms, while the achievement guide separates official achievement text from run-planning advice. Patch 1.01 also fixed the awarding of several campaign-completion achievements, so update before investing in a long modifier run.
Carry only the supplies that support the next route objective. The inventory guide and beginner guide help balance recovery against ammo, weapons, and mandatory tools.
Verification note
Sickness, Hardcore-default, vodka-effect, and Quick Use facts were checked in official Steam Patch 1.01, 1.03, and 1.04 announcements on September 6, 2026. The page intentionally stops where the official information stops.
